During the first visit, we make sure to obtain important background information, like your medical history, and give you time to get to know your doctor.  We accept most insurances.

When to arrive:

For new patients, we schedule your appointment 10 minutes in advance to allow adequate time for you to complete any necessary paperwork. Your initial visit may require up to 60 minutes, depending upon the nature of your problem.

What to bring to your appointment:

Being well-prepared for your appointment will ensure that the doctor has all of the needed information to provide the best possible care for you. It also will help relieve any unnecessary anxiety you may be feeling. Educate yourself on your symptoms by reviewing the content on this site. Please remember to bring the following items to your appointment:

  • Photo ID
  • Patient's insurance card
  • List of current prescriptions and/or over-the-counter medication, including dose and frequency
  • Information about patient's medical and surgical history
  • Recent test results, x-rays, or relevant records
  • Your primary care doctor’s contact information
  • A referral from your primary care doctor (if required by your insurance carrier)

Insurance and Payment Information

You are responsible for co-payments or charges that are not covered by your insurance. If you have questions regarding billing or which insurance plans we accept, please call our office. Questions regarding insurance coverage and benefits should be directed to your employer or insurance company.
